E-Visits

For non-emergent needs use MyChart to complete a symptom-based questionnaire on your smartphone, tablet, or computer and expect a response in about 1-2 business days from your primary care provider (if available) or a provider from our primary care team.

Submit an E-Visit

Use MyChart

Submit a questionnaire during business hours:
Monday – Friday
8:00am – 4:00pm

To submit an E-Visit in MyChart

  1. Select “Menu” then locate “E-Visit” OR select “Messages” then “Send a message” then “Start an E-Visit”
  2. Read the “Terms and Conditions”, click Accept
  3. Submit your information via the provided form
  4. In 1-2 business days a provider who has reviewed your information will contact you via your MyChart account to discuss your condition and determine treatment

E-Visits will be billed to your insurance on file. You may be billed for costs not covered by your insurance plan
