Patient Messaging Information –
Terms and Conditions

Through Stormont-Vail HealthCare, Inc.’s and it subsidiary Cotton O’Neil Clinic’s (collectively Stormont Vail Health), “Treatment Authorization and Related Agreements” and “Assignment of Insurance Benefits, Acknowledgement of Payment Responsibility and Telemedicine Consent” forms, patients are notified of and consent to receive communications regarding their healthcare service and related accounts by regular mail, electronic mail (e-mail), text message and/or telephone (including cell phone).  Stormont Vail Health may also send communications to those individuals specifically identified by a patient. These messages may involve the following information:

  • billing notifications;
  • updates or requests related to your visits and/or scheduled appointments;
  • information related to your MyChart account;
  • a one-time passcode;
  • prescription reminders; and
  • other similar messages related the management of your health care and/or your relationship with Stormont Vail Health.

Patients may opt out from receiving messages through the various forms of communication, as well as obtain additional information or assistance by contacting the Stormont Vail Health Helpdesk at the following telephone number (785) 354-5430 or by email at [email protected].

Special Notice Involving Text Messages:

Patients of Stormont-Vail HealthCare, Inc., and its subsidiary Cotton O’Neil Clinic, its designees, and/or approved agents or contractors (“Stormont Vail Health”), may receive five or six digit number (SMS code) or other text messages regarding: billing notifications; updates or requests related to your visits and/or scheduled appointments; information related to your MyChart account; a one-time passcode; prescription reminders; and other similar messages related the management of your health care and/or your relationship with Stormont Vail Health.

You can opt-out of receiving the above messages text messages by texting “STOP” to any text message you receive from Stormont Vail Health.  Your opt-out request will generate one final text message confirming that you have been unsubscribed and thereafter, you will no longer receive text messages from Stormont Vail Health.  If you want to join again and receive the above-described text messages, simply sign-up through your MyChart account or text “HELP” to a previously received Stormont Vail Health text for instructions.

Stormont Vail Health nor mobile phone carriers are liable for delayed or undelivered text messages. Message and data rates may apply for any messages sent to you from us and/or to us from you. Message frequency may vary.
